Change of Grade: Online

This form is used to change a student's grade after the grade has been submitted and verified at the end of the term.

Course Waiver and Substitution: Online

Submit this form to waive a course from a student’s degree requirements or substitute a course within a student’s degree requirements. Course waivers and substitutions are only made in special circumstances. (Department chair, program director and program coordinator for expressive and creative arts submission and approval only.)

Prerequisite Waiver: Online
