
Online Forms
The forms listed on this page are formatted for submission as online forms through Etrieve and are accessible from any standard internet browser. If you don't have Salve credentials, you won't be able to access the electronic forms.
Access online forms in Etrieve

Student Forms
- Add/drop: Used to add and/or drop a course that you're unable to change through the portal. Permission from the instructor, department chair or program director may be required.
- Change of address/phone: Changes your legal/permanent home address, which is used for mailings such as invoices. (Your local campus address may be changed in My Salve under User Options.)
- Change of advisor: Used to request that your academic advisor is changed. The request is subject to approval by the applicable department chair.
- Change of legal name: Changes your legal name on your academic record. One of the following validation documents must be attached: driver’s license, marriage license, military ID, court order, U.S. passport or federal/state ID.
- Change of major/concentration: Required to add, change, or remove a major or concentration. You must meet with the applicable department chair or program director before you request to make a change.
- Change of minor: Required to add or remove a minor. You must meet with the applicable department chair before you request to add a new minor.
- Course audit: Adds a course to your schedule for audit. No academic credit is granted for audited courses.
- Course authorization: Required for undergraduate students to register for a graduate-level course. Approval from the undergraduate faculty advisor and graduate program director is required before the add/drop deadline.
- Course withdrawal: Used to withdraw from a course with a grade of “W.”
- FERPA release: Authorizes Salve to release academic or financial information to designated persons. Academic information includes grades and academic status, while financial information includes invoices and financial aid awards.
- Grade release authorization: Authorizes Salve to release your grades to a designated third party.
- Graduate and Professional Studies leave of absence: Used for graduate and professional studies students to request a personal or medical leave of absence from Salve. Students should email the graduate academic progress specialist at gradoffice@salve.edu prior to submitting their completed form to sruregistrar@salve.edu.
- Health professions interest (add or remove): Used to request or remove an academic advisor for health professions.
- Incomplete grade request: Used to request an incomplete grade. The form must be fully processed and approved by the instructor before the last day of the semester.
- Registration form: Used to register for courses if you are a non-matriculated student, conditional admit, and/or have an exceptional situation with online registration. Completed forms should be emailed to sruregistrar@salve.edu.
- Special enrollment: Required for all special enrollments, including directed studies, independent studies and thesis. A well-developed course syllabus must be attached, and the form must be fully processed and approved before the add/drop deadline.
- Transfer credit approval: Used to request permission to complete a course at another regionally accredited college or university and transfer the credits to Salve. The form must be submitted and approved before you register for the course.
- Undergraduate pass/fail request: Used to request a pass/fail grade. The form must be submitted to the Office of the Registrar no later than the withdrawal deadline.
- University withdrawal: Used to request to be withdrawn from Salve. Completed PDF forms should be emailed to sruregistrar@salve.edu.
Faculty Forms
- Change of grade: Used to change a student's grade after the grade has been submitted and verified at the end of the term.
- Course waiver and substitution: Used to waive a course from a student’s degree requirements or substitute a course within a student’s degree requirements. Course waivers and substitutions are only made in special circumstances. (Department chair, program director and program coordinator for expressive and creative arts submission and approval only.)
- Prerequisite waiver: Used to waive a prerequisite to allow a student to register for an undergraduate course. (Department chair, program director and program coordinator for expressive and creative arts submission and approval only.)
