Health Records

All new students, including first-year, transfer and international students, are required to complete their electronic health record through Salve's student health portal. Health records must be completed by June 15.

Required Health Forms

  • Immunization record. Upload a signed copy from your medical provider under the "My Forms" tab or automatically import state medical records under the "Immunizations" tab (there is a $20 fee for this service). Required immunizations:
    • Two doses of measles, mumps, rubella (MMR)
    • Three doses of hepatitis B
    • Tdap within the last 10 years
    • Two doses of meningitis MCV4 (meningitis B is not a substitute)
    • Two doses of varicella
  • Medical history (available in the portal to complete electronically).
  • Physical examination (a form from your provider's office is also acceptable). You must have an exam within one year of starting classes. As annual physicals are insurance-based, please submit your most recent physical. Updated physicals may be submitted once they are available. Per the NCAA medical handbook, student-athletes must have an exam within six months of beginning practices.
  • Tuberculosis screening (available in the portal to complete electronically). If you answer "Yes" to any of the screening questions, you must also submit the tuberculosis testing form, which must be signed by a medical provider.
  • Copy of your insurance card (front and back).
  • Medical care authorization form.

How to Complete Your Health Record

  • Log in to the student health portal with your Salve credentials.
  • If you're electing to use the automatic immunization service, follow the registration and payment process in the "Immunizations" tab.
  • Complete the required forms in the "My Forms" tab. Some forms need to be printed and signed by your medical provider. Once completed, attach the corresponding document to each section and submit.
  • You will have a "pending" notification until all forms are properly submitted. Forms are processed manually, and it may take a few weeks for your record to be marked as complete.
  • You will be notified of any missing information through your Salve email.

All forms must be submitted through the student health portal. Forms will not be accepted by fax, mail or email.

ABCs of Healthy Living

All new students must also complete the online course "ABCs of Healthy Living" through Vector Solutions. You will receive login credentials and information on how to access this course once you have your Salve credentials.
