Online Forms for Faculty

This webpage contains forms for Salve Regina faculty, which are formatted for submission as online forms through Etrieve. These forms are accessible from any standard internet browser, and all forms require login to the portal.

Faculty Forms

Change of Grade: Online

This form is used to change a student’s grade after the grade has been submitted and verified at the end of the term.

Course Waiver and Substitution: Online

This form is used to waive a course from a student’s degree requirements or substitute a course within a student’s degree requirements. Course waivers and substitutions are only granted in special circumstances. (Department chair and program director submission and approval only.)

Prerequisite Waiver: Online

This form is used to waive a prerequisite to allow a student to register for a course. (Department chair and program director submission and approval only.)
