July 15 - To faculty and staff: Reminder: Submit your vaccination status

If you haven't submitted your vaccination status yet, don't miss out on your chance to win a $500 Amazon or Visa gift card. Employees who provide their vaccination status by Monday, July 26 will be entered to win of two $500 prizes. All current employees are eligible, although adjunct faculty must be teaching during the fall 2021 semester in order to qualify.

Process to Submit Proof of Vaccination or Request an Exemption

Faculty and staff may provide proof of vaccination securely and confidentially via the employee submission form. No other medical information should be provided. Your vaccination record must show that you received one dose of Janssen (J&J) or both doses of Moderna or Pfizer in order to enter and win.

Please use the exemption request form to request an exemption based on a bona fide medical or religious reason. Forms should be printed, completed and returned the Office of Human Resources via mail or in person to ensure confidentiality.
