Housing Accommodations Form

Housing accommodations are based on medical evidence of need as determined by ADAAA and FHA policies and guidelines, and are determined on a case-by-case basis by the housing accommodations review committee. To qualify as an individual with a covered disability as outlined by the Americans with Disabilities Act Amendments Act (ADAAA), the student must have a current condition that substantially limits a major life activity, and the accommodation must be necessary and reasonable. A diagnosis in and of itself does not automatically qualify an individual for housing accommodation(s).

To request a housing accommodation, complete the form below and supply/attach all required documentation. All applications and documentation provided will be thoroughly reviewed, but accommodations are not guaranteed. Incomplete applications cannot be reviewed.

The preferred timeline for new student housing requests is June 1 for the fall semester and Dec. 1 for the spring semester. The preferred deadline for returning students (rising sophomores, juniors and seniors) is March 31.
