Info for Students

Please Note: You must be medically cleared with an annual physical before you begin varsity practices each season at Salve Regina University.

Please read through all instructions before completing any paperwork and print legibly with blue or black ink and sign all forms where indicated.

Freshmen/Transfers/New Athletes

All Athletes: NCAA Banned Substances

1. If a student-athlete is taking medications that are on the NCAA banned substance list, the prescribing physician must provide documentation to the sports medicine staff that all other non-banned alternatives have been exhausted prior to administering the drug. Please review the following information regarding banned substance exemption, or contact the sports medicine staff.
2. If the student-athlete takes prescribed ADHD medication, please follow the instructions on the NCAA web site.
3. The NCAA recommends that division III student-athletes confirm sickle cell trait status during a medical examination. We have included several questions in the medical forms regarding sickle cell trait, including a test results portion in the physical form that your physician must fill out. It is recommended that you confirm sickle cell trait status during your physical, however this is not required. Please read a release from the NCAA for more information.

Deadlines

• If you participate in a fall sport (cross country, field hockey, football, soccer, tennis, volleyball) your completed paperwork must be received by July 15.
• If you participate in a winter sport (basketball, ice hockey), your paperwork must be received by August 1.
• If you participate in a spring sport (baseball, lacrosse, softball, track & field), your paperwork must be received by August 15.
