Info for Students

Please Note: You must be medically cleared with an annual physical before you begin varsity practices each season at Salve Regina University.

Please read through all instructions before completing any paperwork. Please print legibly with blue or black ink and sign all forms where indicated.

A. Freshman/Transfers/New Athletes/Tryouts

  1. There are two sets of forms that varsity athletes are required to complete. The first set is for campus health services and should be completed upon acceptance to the university. THE SECOND SET OF FORMS IS FOR SPORTS MEDICINE, SEE INSTRUCTIONS BELOW (A2). The forms for health services are different from sports medicine forms and should be submitted ONLY to health services. These are the forms that are required by health services:
  1. Please fill out the Athletic Participation Form. Please answer every question. If you have answered 'yes' to any question, please provide an explanation in the spaces provided. This form is different from the health form that you return to health services. Please send the Athletic Participation Form to SPORTS MEDICINE ONLY.
  1. Please fill out the HMO Referral Form if your insurance has an HMO. This form is necessary to expedite any additional treatments from the Salve Regina Sports Medicine Staff. Please return this form with the Athletic Participation Form.
  1. If the student-athlete is under 18, a parent or legal guardian must sign all forms.
  2. If you have had any significant injury or surgery in the past 12 months, please include a written release from the treating physician stating that you are cleared to participate in intercollegiate athletics.
  3. Make a copy of all paperwork for your files. Please forward the Athletic Participation Forms and the HMO Referral Form to Sports Medicine (address below). Faxes are not accepted.

B. Returning athletes ONLY

  1. Please fill out the Returning Athlete Participation Form.
  2. If you have had any significant injury or surgery in the past 12 months, please include a written release from the treating physician stating that you are cleared to participate in intercollegiate athletics.
  3. Please make a copy of all paperwork for your files. Please send the original copy to Sports Medicine (see address below). Faxes will not be accepted.

C. For all athletes: NCAA Banned Substances

  1. If a student-athlete is taking medications that are on the NCAA banned substance list, the prescribing physician must provide documentation to the sports medicine staff that all other non-banned alternatives have been exhausted prior to administering the drug. Please review the following information regarding banned substance exemption, or contact the sports medicine staff.
  2. If the student-athlete takes prescribed ADHD medication, please follow the instructions in this link.

Deadlines

  • If you participate in a fall sport (cross country, field hockey, football, soccer, tennis, volleyball) your completed paperwork must be received by July 15, 2009.
  • If you participate in a winter or spring sport, your paperwork must be received by August 1, 2009.

Athletic Training Room Rules

  1. The Athletic Training room is co-ed and all athletes must be properly dressed.
  2. NO cleats or spikes in the athletic training room.
  3. Do not bring clothes, bags, uniforms, or any equipment into the athletic training room.
  4. Modalities are to be administered by trained staff members only. Whirlpools are to be filled by a member of the athletic training staff ONLY .
  5. All equipment and supplies must remain in the athletic training room unless they have been signed out through an athletic trainer or staff member.
  6. All athletes are required to sign in before receiving treatment. Treatment logs are open to all coaches for review.
  7. Athletes are responsible for reporting all injuries and illnesses.
  8. Athletes are to make appointments with the athletic training staff. Failure to show for the appointment will be reported to coaching staff.
  9. No horseplay, loitering, disrespectful behavior or improper language will be tolerated in the athletic training room.
  10. Absolutely no one is to be in the athletic training room when doors are locked and/or lights are shut off.
  11. No food, beverages, or tobacco allowed in the athletic training room.
  12. Under no circumstances are student employees to administer treatments unsupervised.
  13. Being in the athletic training room is not an excuse for missing or being late for class/practice.
  14. You MUST shower before treatment or rehab.
  15. You must shower before going in the whirlpools after practice. NO open cuts, wounds, or sores allowed in the whirlpools - no exceptions to this rule.
  16. No cameras or recording devices of any kind allowed in the athletic training room.
  17. DO NOT remove towels.
  18. No shoes allowed on treatment tables at any time.
  19. If you lay on a table and are sweaty/dirty you must clean the table when you are done.
  20. Dump ALL ice in the WHIRLPOOLS, not the sink.
  21. Tape, gloves, and other medical supplies are for injuries ONLY, not equipment, uniforms or personal use.
